Whole-person care looks at the physical medicine and behavioral care medicine needs of a patient consecutively. This provides the following benefits:

- Consultations on care plan with psychiatrist and pharmacists to assist in monitoring and assessing of progress

Our care management platform works alongside each care providers EMR. We track all patient interaction events and document amount of time care managers spend working with patients and their care team. For most behavioral care plans we utilize a targeted care approach utilizing measurable clinical assessment tools with a target of 50% reduction (improvement related to reduced scores).
Our patient mobile application platform tracks user usage and is also utilized for assessment documentation and care plan compliance such as medication usage/compliance.
This collaborative care program is administered by the employees primary care provider and any cost sharing relates to primary care co-pays, if applicable. Total employee cost-sharing will vary based on each employers plan design.
A referral to a specialist behavioral or substance use professional would then fall under the specialty care benefit construction of the employer benefit plan design.
There are two approaches:
- Proactive assessments are sent to each employee, typically as part of EAP program. If behavioral or substance use care is needed, an immediate referral is made to a PrometheusPX network provider. These care providers are trained to identify a patients primary care provider and approach for implementing a collaborative care program, including the behavioral care manager. Once complete, patient consent for participation documented and care provided.
- Employer provides data file for employees with historical behavioral or substance use needs. Proactive assessments are sent to these employees and results managed by care navigator. Patient primary care provider identified and approached for collaborative care integration. A care team of psychiatrist, pharmacists and care manager are assigned to each primary care group and service provided to the employee.
It is important to note, patients have immediate access to outpatient therapy, outpatient psychiatric therapy, inpatient substance use and inpatient mental health providers should the need arise.
