Medication Therapy Management
There is a need among providers for a proactive, unified care approach characterized by the seamless orchestration of digital and in-person interactions across communication channels. When combined with our ongoing development of data analytics and artificial intelligence to develop predictive models for certain therapies, medication side effects, individual characteristics such as metabolism and potential occurrence of adverse events, we can efficiently assist physicians in developing precision medicine protocol for patients.
Genome Assessment
One of the byproducts of our patient assessment includes the development of Personal Medication Record (PMR) and a Medication-Related Action Plan (MAP).  This includes the results of PGx assessments as well as other drug-drug interaction analysis.   The value of pharmacogenomic information is increasing rapidly as researchers identify more drugs where genetic variations have serious therapeutic implications. Pharmacogenomics (PGx) is a proven tool in clinical practice, ensuring that medication therapies are effective and safe for patients while avoiding wasted time and resources. In a recent study, St. Jude found that “about half of the hospital’s patients would eventually need to be treated with a drug where pharmacogenetics is relevant.”
Prescriber Support
We have successfully integrated a pharmacists-led Medication Therapy Management protocol as part of our data analytics and patient care process.  To ensure success of our process with each client, we provide two solutions for supporting integration:
