The objective was to build a high performance, multi-disciplinary network of inpatient, outpatient, and community based organizations (CBOs), creating a circle of support around each patient care needs.
Success elements include:
- The use of advanced screening and assessment tools
- Mitigation of social and medical obstacles to treatment compliance
- Extensive network of connected professionals for quick access
- Coordination and collaboration at all levels of care
Behavioral health care network participants agreed to measure patient care progress singularly and jointly toward process performance goals:
- Reduction in behavioral health 30-day readmissions
- Decrease in avoidable bed days (post-medical clearance)
- Acceptance of urgent needs patients within 48 hours of referral
A Supportive Connected Community
PRMcare, a novel & scalable high- performance network of care that organizes routine, ED/short term acute care for behavioral health and substance use patients. PRMcare offers incentives for providers to establish collaborative care programs with primary care providers for enhanced care delivery and patient satisfaction.
